Recently get a DWI, DUI, or OWI? Needing an eval for family court or custody matters? LiveWell offers chemical dependency or substance use evaluations. These are independent assessments with individualized recommendations for every client. We evaluate personal history, past and current substance use, risk of continued use or relapse, mental health, coping skills, healthy supports, and recovery environment.

Appointments are 45-55 minutes in length and primarily scheduled via telehealth or online from the comfort of your home. For those that prefer an in-person appointment, there is some availability for this but times are booked quickly.

There is no obligation to continue services outside of the assessment and you will receive written recommendations within 48 hours.

Chemical dependency evaluations or substance use assessments are $120 per assessment. If you need the assessment scheduled and recommendations within 24-48 hours, it is an additional $50. Payments can be made via most major credit and debit card holders, cash, or Venmo. Payment is due immediately after the assessment.
